What is PACE?

PACE (Program of All-Inclusive Care for the Elderly) is a nationally recognized model of care that helps eligible older adults remain in their homes and communities while receiving comprehensive healthcare and support services.

We bring together healthcare, transportation, social services, rehabilitation, and home-based support through one coordinated care team.
